CONSUMER BANKRUPTCY INTAKE FORM

To further assist you, we offer a downloadable version of our client intake form. This form requests specific information about your circumstances, which will allow our attorneys to better analyze your financial situation. This intake form is used in all of our consultations with our clients.

Many clients are more comfortable filling out the intake form in the comfort of their own homes.

Information provided by the client on this intake form is confidential.

Information provided by the client will also be used in the eventual preparation of a bankruptcy petition, which will be filed with the bankruptcy court.

OBTAINING A PERSONAL CREDIT REPORT

If you believe that you will be unable to provide our office with up-to-date information, you may wish to consider downloading a copy of your free annual credit report from www.annualcreditreport.com. You are entitled to one free credit report per year from the participating credit reporting agencies. For security purposes, we would recommend that you do not permit anyone else to utilize the www.annualcreditreport.com website to obtain the information on your behalf.
